Notifiable Events form

This form is for Commonwealth Teaching Scholarships (CTS) Program recipients to notify the Australian Government Department of Education (the Department) of any changes to their circumstances that may impact the details of their Scholarship Agreement.   

As stated in the Scholarship Guidelines (11.5), a CTS recipient must ensure that the Department is notified of significant events and changes to personal circumstances, within 28 days.

Please note, you will be required to provide certified or official evidence for changes relating to your legal name, higher education provider and/or initial teacher education course. The CTS Program Team cannot accept unofficial transcripts or screenshots for changes relating to study. If you do not have official documents, you will need to contact your higher education provider to request this.
